Imiquimod-Induced Interleukin-1¥á Stimulation Restores Skin Barrier Abnormalities by Topical Calcineurin Inhibitors

Abstract
Topical calcineurin inhibitors(TCIs) have antiinflammatory and immunosuppressive effects. Our previous study showed that barrier recovery was delayed after acute barrier disruption in the skin treated with TCIs. In hairless mice, epidermis treated with TCIs showed the decrease of lamellar bodies(LB) number and secretion, lipid synthesis related enzymes, antimicrobial peptides (AMP) and interleukin 1¥á(IL-1¥á) expression. IL-1¥áis an important cytokine to improve barrier function, LB structure and lipid synthesis in keratinocytes. The aim of this study was to evaluate the efficacy of imiquimod, an IL-1¥áinducer, in the skin treated with TCIs. In humans, topical tacrolimus was applied twice a day for 5 days. Then topical imiquimod and vehicle was applied individually after tape stripping. Topical
imiquimod improved the barrier recovery than vehicle. In hairless mice, topical tacrolimus was applied twice a day
and topical imiquimod was applied once a day for 4 days. Topical imiquimod improved epidermal permeability barrier homeostasis in comparison to vehicle. Imiquimodtreated epidermis showed the increase of LB number and secretion and lipid synthesis related enzymes such as HMG CoA reductase, serine palmitoyl transferase and fatty acid synthases. Also imiquimod increased the expression of AMP(CRAMP, mBD3). These results provide a possibility of IL-1¥áinducer to
prevent barrier dysfunction in the skin treated with TCIs.
